Good Faith Estimate

Good Faith Estimate is intended to provide you with an estimate of the charges you'll incur at Motiv Method Physical Therapy & Performance with the information at this time. Uninsured and self-pay clients are entitled to Good Faith Estimates as of Jan 1st, 2022 under the No Surprises Act.  

Clinic Fee Structures   

Your treatment will consist of an initial evaluation. The initial evaluation ranges from $89-$270.

On the initial evaluation, your Doctor of Physical Therapy will go through a thorough assessment to diagnose and uncover the root cause of the issue. You will receive some treatment and exercises specific for your situation on the first initial evaluation.

At the end of the initial evaluation, your Doctor of Physical Therapy will walk you through the exact steps and solution to resolve your issue and get you back to your goals and what the recommended treatment plan will be to achieve the goals.  

Performance Physical Therapy Programs 

$500-$12,000 

Care Credit, HSA (health savings account), and FSA (flexible spendings account), every major credit card may be utilized for services seen at Motiv Method Physical Therapy & Performance. 

Payment plans are available upon individual request. Visits on packages will expire within 6 months of purchase date. All sales are final and that the company has a no-refund policy. Any funds remitted are non-refundable in all circumstances.
